Amory G. C. Grijaldo
AGACNP-BC
I am an ANCC board-certified nurse practitioner specializing in the care of the adult and geriatric population. I am the founder of the Adult Geriatric Care Group. I completed my bachelor’s in nursing in 1994 at the Pontifical and Royal University of Santo Tomas in the Philippines, and my master’s in nursing specializing in adult and geriatric care in 2014 at the University of Arizona. I have extensive experience in managing chronic conditions, including chronic kidney disease while employed at Southwest Kidney Institute and Mayo Clinic. I am fluent both In English and Filipino (Tagalog).
Razel E. Hernandez
FNP-BC
I am an AANP-board certified nurse practitioner with over 30 years of experience in the medical field. I have completed my bachelor's in nursing at Colegio de San Lorenzo Ruiz de Manila in 2002 in the Philippines and my master's specializing in Family Medicine in 2019 at Grand Canyon University. I am knowledgeable and have an extensive experience managing various types of wounds. I am certified by the National Alliance of Wound Care and Ostomy. I have been an Arizona resident for the last 15 years. I am fluent both In English and Filipino (Tagalog).

About Us
At Adult Geriatric Care Group (AGCG), we provide quality and in-person and virtual ambulatory medical services to both the adult and geriatric population at home and at congregate care facilities (e.g., group homes, assisted living, and skilled nursing facilities).
