How it works
Set an Appointment
Call or click the request appointment button to request for an in-person (house call) or virtual visit. Your appointment request will be reviewed by a provider, and you will receive an email or a text message confirming your appointment.

In-Person Visit
Schedule your appointment for your preferred day and time. You may be required a copay at the time of visit. You will receive an ETA (expected time of arrival) via phone call or text message on the day of the visit. We recommend finding a safe, quiet, and private location in preparation for your visit. It should provide adequate space for an exam. If you have pets, we recommend placing them in a secure location before the provider arrives.
Virtual Visit (Telehealth)
After your visit, your prescription will be sent to your local pharmacy and a visit summary will be forwarded to you and your primary care provider (PCP). You will be able to access your visit summary and itemized receipt through your patient portal.

Adult Geriatric Care Group provides in-person and virtual (telehealth) ambulatory medical services to both the adult and geriatric population at home and congregate care facilities (e.g., group homes, assisted living facilities, skilled nursing facilities). Virtual visits can be performed through your smart phone, tablet, or personal computer from any location of your choosing.
Adult Geriatric Care Group is credentialed with most insurance providers. You may be required a copay at the time of visit. We recommend speaking with a staff member for more information.
We currently serve Maricopa County which includes the cities of Phoenix, Mesa, Scottsdale, Chandler, Gilbert, Tempe, Glendale, Surprise, Buckeye, Goodyear, Avondale and Sun City.
You will see a nurse practitioner during your medical visit. Nurse practitioners are highly, trained board-certified medical professionals. They are licensed to provide medical care independently in states with full practice authority for nurse practitioners which includes Arizona.
